Hello. I am hoping to get some advice/direction on our next steps. We have been trying to concieve naturally for the past four years. This has been after a failed vasectomy reversal. No sperm in the ejaculate.. Our next step is most likely going to be aspiration/icsi/ivf. What should our next steps be. We are paying out of pocket because my husband has a zero count (uncucessful reversal) and want to know if we should go for a consult first at the fertility clinic or should he be seeing a urologist first? After being sterile for 8 years is this a good option for us? Thanks so much for any help you can provide.

A re-do on vasectomy reversal is not likely to work. Go see a Reproductive Endocrinologist for IVF with ICSI. Frequently, RE's collaborate wit urologists for sperm aspiration. Either the 2 will need to co-ordinate procedures, or the sperm will need to be frozen before the IVF process.
Good luck.
